triple tapped mains/undersized branch circuits

Found this distribution panel in a garage today.
if you look closely you can see that the main wire on the right side had 3 wires terminating under one lug.  One wire is a 12 gauge wire, connected to 50 amps of current.

If something were to go wrong that wire would melt and likely start a fire.  You never know what you will find when you open an electrical panel.

 

What happens when a wire is too small for a breaker?  It is kind of fun to watch!
